If you wish to register with Abbey Medical Group you will need to call in to the surgery.
You will be asked to complete a new patient registration form as well as a questionnaire and an ethnicity form. This can only be done in person. You will need to bring with you NHS number if you have it as well as details of your previous doctor. You will be given this document when you call in to the surgery.
The new patient questionnaire and the Ethnicity form can be downloaded below and you can complete before you call at the surgery. This will speed up the process for you.

The practice uses the connecting for health GP2GP service which means that once you have registered, providing the system is compatible, we can electronically receive your clinical records from your previous GP within a few hours. This is a huge benefit to you and your new GP.
