When you attend your appointment you need to bring:
- For New Zealand: Form NZIS1007
- Three passport-sized photographs for medical and radiography forms.
- Your passport.
- Any prescription glasses or contact lenses you use.
- Any hospital or doctors letters relating to any serious illness you have had.
- A list of any repeat prescription medication you are on.
If you have any significant illnesses e.g. diabetes, heart disease or high blood pressure, or have had recent surgery, the Immigration Authorities may ask for further information. You can discuss this with the examining Doctor during the medical, and we will provide a letter to take to your GP or Specialist for a report. You can request this information in advance and bring it with you to save time, if you wish.
Please note that you should drink plenty of water prior to your appointment as a urine sample will be requested. Ladies should avoid booking appointments during their menstrual period.
